MEDICAL PLANS AVAILABLE IN OKLAHOMA

1. American Medical Access Group Plan

MUST BE EMPLOYER SPONSORED

	Employers have 3 plan options to choose from, However, only one plan can be selected for your group.
	Available for groups with 3 or more lives (20% of group participation is mandatory). If your company has under 51 employees, the participating employees must join the NACD at a cost of $10.00 per month, and will receive the additional NACD benefits.
	If your company has over 51 employees, then the participating employees do not need to join the association.
	Fully Insured By Companion Life Rated A+ (Superior)

2. American Medical Access Plan For Individuals

	Must join the NACD (National Association of Consumers Direct) $10 monthly NACD association access fee.
	Includes many extra benefits through the NACD.
	Available to any member, member and wife, or member plus 2 or more
	3 plan options to choose from

NOTE: To maximize your prescription drug benefits, we recommend that you also purchase the ABC-Rx Prescription Drug Card. The prescription coverage included in the indemnity plans are only a $20 reimbursement per prescription, up to the amount allowed in whichever plan you chose.

This low cost prescription card if purchased with the indemnity plan is only:

	Member - $6.00 per month
	Member plus 1 - $8.00
	Family - $9.00
